I haven't watched the most recent (4-24) eppy yet. It's on the DVR. It was
my birthday yesterday & I just didn't have time. But judging by the lack of
posts, I'm guessing no one else has either! Sheesh! This is sad, sad, sad.

We actually did. (we haven't been faithful). It was all Abby all the time.
She saved the day, of course. And, yet, another baby was birthed in the ER.
Baby saved the day. After umpteen seasons tpb just don't get the fact that
most of us out here know that pregnant women brought in the ER go directly
to L & D. When will they stop trying to re-create LLL?

I watched it before work this morning, and, as Vetgirl said, it was
Abbytastic. I must say MT is looking mighty worn these days.

The whole gang, including Coburn, needs to be reprimanded for their unkind
and insensitive remarks to the woman with the passel of kids. Her religious
beliefs are nobody's business and telling her she must be nuts for having so
many was plain old rude.

I noticed the writers seem to have forgotten that Luka was back in Chicago,
waiting for Abby to come home when his father died, not at his bedside in
Croatia. Or maybe we are supposed to think Luka lied about that in his job
interview for no apparent reason. Of course, with the sanctification of
Abby getting underway last night, I suppose the demonization of Luka is
inevitable, so maybe we are supposed to think he's a pathologic liar.

I have no idea what the big deal is about the baby's hand being over it's
head at delivery. It's called a compound presentation and it is delivered
just like any other kid. I do not know what all the 'ooohing' and 'ahhing'
was about; nor do I get what special thing it was that Abby had to do to
deliver the kid. By number 13, the trail is pretty well blazed and they pop
out without much assistance, IMO.

I also don't know what was up with all the insistence that the ER wasn't
equipped to handle the case. Jody O'Brien had an abruption, a shoulder
dystocia and a cesarian, for goodness sakes; this was just a regular vaginal
birth. Hell, just last year, Abby sawed some chick's pelvis in half to do a
delivery and nobody batted an eye. I suppose TPTB weren't imaginative
enough to come up with a decently heroic scenario, but were desperate to
start the redemption of their Special Princess, so they figured that having
her coworkers look on her with awe and admiration and praise her to the
skies for not really doing much of anything was the way to go.

Otherwise, not much else to say about the eppy. Looks like they are setting
up all of the major crises for the season finale cliffhanger and, by now, we
can figure out where each character is heading from a mile away.

Is there any circumstances in real life where they would actually do

that?
Yep, but, here in the US, it has probably happened less than a half dozen
times in a decade. It's considered to be a last resort treatment for
shoulder dystocia (when the baby's shoulder gets stuck behind the mom's
pubic bone after the head has delivered. Huge emergency). There are about
a dozen other things that would be tried first, any which of one is far more
likely to work, most of which Abby didn't even try before pulling out a
knife and sawing through the pubis symphysis. I've certainly never seen a
symphysiotomy done ever and I've been practicing OB for 26 years, seems
extremely unlikely that Abby, who hasn't been an OB nurse in almost a decade
would have ever seen the procedure, let alone known how to do it.

BTW, Abby did the whole thing using just a little local anesthesia, probably
less than your dentist would use to fill a tooth. Yet, her patient
seemingly didn't feel a thing and, just minutes after delivery, was sitting
up, holding her baby and cooing at it. In real life, the patient would be
in a significant amount of pain both during and afterwards and would more or
less have a pelvic fracture which would be disabling for weeks if not
months. She'd need crutches and extensive therapy for recovery and runs the
risk of chronic pain and pelvic instability; which is why most docs wouldn't
even consider doing it unless every other possible option was exhausted.

Symphysiotomy to treat obstructed labor is done much more commonly in the
third world, where the facilities are not available for doing cesarians. I
had a med school friend who did some volunteer work in Africa and he saw a
couple of them done for women who had been in labor for days and were at
risk of dying. The area where he was working didn't have OR's, anesthesia,
IV's, antibiotics or blood transfusions available, so the risk of dying from
a cesarian is very real. He said that most of the women were unable to
spend the weeks off their feet that a symphysiotomy requires and therefore
healed poorly and were left with an unstable pelvis that caused them to
wobble when they walked. He said that when they were travelling through the
villages, they could pick out the women who'd had a past symphysiotomy by
their characteristic gait.

Is there any reason to do it where a C-Section wouldn't be a more
desirable choice?

Also wouldn't the patient need the pelvis replaced since i'd assume
that after you do what Abbey did you'd have a patient who'd
be very apt to fall and not be able to walk?

I'd have to assume that once you saw the pelvis in half you've
got a patient who is wheel chair bound for the better part of
a year.

Were the kids at least clean and kempt? (Eh. When I worked at the
library, we had a patron who always brought along her pack of feral
ragamuffins... walking advertisements for tubal ligation, the lot of
'em.)

It's not like they didn't leave the door open for it with that "how my
family died" cock-up in S6 and the weird storyline with the guy who
thought he had kids, which I still think would have been an
interesting direction to go.

Hey! How is your friend?
Just finished week #6 in the ICU. She is trached but on the vent only at
night, trach masked during the day. Stomach tube for nutrition and is
finally trying to eat some. She's up in a chair about 4-6 hours a day which
is great for the lungs since she has to use the whole lung to breath instead
of just the top half. Doing OT and PT to get moving. Not walking yet.
Kidneys are much better but still have to do dialysis every few days to get
off some fluids. Lung transplant doc is considering moving forward with
getting her on the transplant list at the top. Team meetings every Tuesday
to consider that. Once that happens it would be a 2 day to 2 month wait
because she's small. The kidney docs want to see more improvement elsewhere
before they consider also tranplanting kidney(s).

So it's good news, relevant to where we were 6 weeks ago. CF stinks. Our
girl is a fighter.

Awwww, bless her heart! She IS a fighter! :-) I wish & pray all the best.
BTW, how is her family holding up? It has to be hard on them, as well. How
is her son?

This is nothing new to them. They're doing as well as can be expected. They
are trying to maintain as normal a schedule for their son as possible with
school etc. He sees her on the weekend if he's not got a cold or illness.
Sometimes I have 4 boys here instead of three, but what's one more!

At least for now, anyway (WaT ended a mite early, so I switched over
to NBC just in time to see Luka looking all Byronic, which is always a
lovely view). I figure either he'll realize (after getting a Very
Special Patient) that *gasp* he just can't live without her (and
decide to forgive'n'forget) or she'll get to be all heroinic and
martyry as the poor abandoned single mother.

Or... he'll get hit by a bus. Not exactly ruling it out.
